You are joining the program with a family or youth from Bellefaire JCB.

This agency places children & youth in foster or kinship care with appropriate temporary living situations, trains foster parents in therapeutic care, and has many additional services for families. This agency has oversight of the foster home or youth you are matching with, and any Cleveland Angels volunteers who will be in person consistently with the family/youth must complete their agency-specific requirements.

Bellefaire JCB AGENCY Requirements

1. Download and complete a form of Consent for Ohio's SACWIS Alleged Perpetrator Search(OSAPS) and return to the agency.
2. Sign Up for your Program Orientation with Cleveland Angels (if you have not already done so).

In addition to the Sterling background check you've completed, this agency requires proof of identity, and the 'Consent to Ohio's SACWIS Alleged Perpetrator Search' form.

Please download & complete the form linked here, and share it to the contact below along with a copy of your Driver's License/State ID, and your Social Security Card. These documents are required to verify your identity.

*If you prefer to deliver your needed document copies in person to Bellefaire, let your case manager know and they will give further instructions.

Please make your Cleveland Angels Case Manager aware once you have submitted this documentation.

NOTE -- A Program Orientation with your Case Manager is important to complete before you begin. Orientation covers things like Foster Care 101, the way trauma affects the brain & behavior, the bounds and expectations of the program you're joining, and more. You'll have an opportunity to ask any and all questions!
