You are joining the program with a family or youth from Ohio Guidestone.

This agency places children & youth in foster care with appropriate temporary living situations, trains foster parents, works with youth taking part in a residential treatment program (living in a therapeutic group environment) and provide several types of therapies. This agency has oversight of the foster home or youth you are matching with, and any Cleveland Angels volunteers who will be in person consistently with the family/youth must complete their agency-specific requirements.

1. Complete The Alternate Caregiver Packet
2. Complete the Fingerprinting through 'Field Print' as defined on Page 2 of the packet.
3. Send:

• Your completed & signed packet
• A copy of your Driver's License
• A copy of your Driver's Insurance
• A copy of your Social Security card

4. Sign Up for your Program Orientation with Cleveland Angels (if you have not already done so).

Please complete these steps by the date in the email you received, and reach out with any questions!

Ohio Guidestone requires any volunteer with consistent contact with children to complete paperwork to be considered an "Alternate Caregiver," and to complete fingerprinting. Being approved as an 'Alternate Caregiver' means only that you are approved to spend time alone with the child/children up to 8 hours, and are approved by Guidestone to babysit, drive the children, etc. This does not approve you to have overnight stays or babysit overnight. If you have questions about why this paperwork is required, please ask your Case Manager, and they will explain further!

A few things to note:

• Guidestone understands the program you are joining as a Cleveland Angels volunteer! The packet will refer to you as "parent" and "alternate caregiver" in some sections, but completing this packet in no way implies that you are becoming a foster parent or are under any obligation to 'care for' the children ongoing. It is simply a safeguard!
• All people who will spend consistent time with the children must complete this packet. If two volunteers live in same household, you can use one form and complete it as "Applicant 1" and "Applicant 2."
• You only must fill in the highlighted sections of the document- detailed instructions are on page 1!

4. Let your Cleveland Angels Case Manager know you have submitted all agency documentation!

NOTE -- A Program Orientation with your Case Manager is important to complete before you begin. Orientation covers things like Foster Care 101, the way trauma affects the brain & behavior, the bounds and expectations of the program you're joining, and more. You'll have an opportunity to ask any and all questions!
